 The forms available online from Clay-Union Electric can be found below. The Feedback/Comments/Inquiry Form can be filled out and submitted online. The Application for Electric Service form, the Budget Billing Application and the Direct Payment Options form for security reasons needs to be printed, filled out, signed, and sent back to us via US postal mail or brought into our office.
